Thank you for your interest in registering! We will be notified as soon as you have registered via the website. We will then register you within a few days or you will be put on the waiting list. When the registration has been processed, you will receive a confirmation email (also keep an eye on your spam box!). From that moment on we are your doctor. Then do not forget to unsubscribe from your old GP so that we can receive your medical file.

You can register at Huisartsenpraktijk Oostenburg when you live or will soon be living 7 minutes bicycle drive from the practice location.
People living together with already registered patients will be accepted immediately, except when the household has moved to an area more than a 7 minute bicycle drive from the practice
IMPORTANT:
- The practice makes extensive use of the patient portal in its communications and a Dutch phone number is required to access the patient portal. We therefore strongly advice not to register if you do not have a Dutch telephone number.
- Please fill in a separate form for each family member.
- Children also need to have a unique email address to create their own portal. This may be an alias of another email address of a caretaker. The phone number may be shared with multiple family members.
- Our system is unable to send a message confirming that your form has been received correctly. Once we have checked your details and your registration is final, you will receive confirmation of this by email
- If you have a question about the registration, feel free to ask via the ‘FAQ’ page and please do not schedule an introductory talk, as these talks are meant for accepted patients.
Practical Information
In the first phase of the practice, a unique collaboration was chosen with the Inntel Hotel, Landmark in Oostenburg. Patients can be seen in a hotel room that has been transformed into a fully equiped consultation room. Patients report to the hotel reception and receive a pass to take the elevator to the 12th floor where chairs are placed in front of the practice room.
Consultation of patients with a Dutch health insurance are directly declared at the health insurance.
